Include your video submission in VHS, DVD, MiniDV, or BetaSP in NTSC format ONLY. Label both the medium and its container (in English, please) with the title of your piece, your name and your contact information. Remove the write-enable tab from any video entries. Be sure to include the official Entry Form, signed and filled out completely, with your submission or it will not be accepted.
